Parent Financial Commitment Form (below) must be completed and returned by March 1, 2013, along with a registration fee of $165. Registration Fee is $500 after March 1.

Tuition Adjustment Form & Instructions:
To participate in the tuition adjustment process for your family, complete the Tuition Adjustment Application below. Please complete all questions, date and sign the Tuition Adjustment Application and return to the school office by March 1, 2013 with a copy of your completed and signed Federal Income Tax return for 2012 (or most recent). For the adjustment system to be fair to all families, re-enrollment applications for adjustment MUST be received by March 1, 2013.
